Physio treatment in ankylosing spondylitis
by Jonathan Blood Smyth
Ankylosing spondylitis is a disease of inflammatory arthritis or spondyloarthropathy, classified as reactive arthritis, bowel disease arthritis and psoriatic arthritis. Underlying relationships between these diseases are complex, but they are connected by enthesitis (inflammation of the ligament / bone junctions) and for possession of HLA B27 in white blood cells. The process of enthesitis at the edges of the board may cause fibrosis and ossification then the area (bone formation).
AS is the most common of the spondyloarthropathies and its incidence varies the presence of HLA B27 in the population, being less common in the tropics and more common in northern European countries. 0.1 to 1.0% of people are affected, but this varies with latitude and is more common in Caucasians. About 1-2% of people with the HLA B27 gene but in fact develop as this becomes 15-20% risk if they have a first degree relative with the disease.
Three men to each woman is the proportion of patients with ankylosing spondylitis in patients female may have much less obvious symptoms and thus lost from diagnosis. Young men are the most common group present in most to consult a doctor before age 40 and up to 20% before the age of sixteen. 25 years is the average age of someone falling to the symptoms and is rarely found a diagnosis of AD in a person over fifty years. It's easy to overlook, as it may seem mechanical back pain if not careful. When asked how are in the morning, a typical response is very rigid.
Ankylosing spondylitis has similarities and differences other than the much more common back pain:
Morning stiffness in the lumbar spine, which lasts at least 30 minutes or more exercise improves back pain and stiffness of rest pain worsens stiffness and pain is usually worse in the second half of the night, after a rest period, the peripheral joints are affected in 30 to 50% of patients Fatigue is common affecting AS has systemic inflammatory nature which may include malaise, fever and weight loss.
Review of physiotherapy of the spine vertebral patient often discovers ranges significantly reduced spinal movement than normal, perhaps with a reduction of lumbar lordosis and a thoracic curve major. The neck movements can also be limited in the later stages and a reduction in chest expansion noted, because of rib involvement joint. Peripheral symptoms occur in about one third of patients and the physio is palpable tender areas in search of evidence of enthesitis at the insertion Achilles tendon and plantar ligament of the foot. These are areas of high mechanical stress and commonly affected.
Postural analysis of the patient, is the first few notes physiotherapist after subjective examination, recording of abnormalities of the spine, knees bent, shoulders rounded or inserted head posture. The ranges of motion of the cervical, thoracic and lumbar spine was measured and a battery of standard measures allows assessment progression of the disease. The hips or other peripheral joints are affected, also to be measured, with testing likely sites where the physio entheses is likely to be painful and inflamed. If the disease is active, the patient may also have joint effusion and malaise may appear, be sweating and have not slept well.
Physical therapists will focus on the treatment of inflamed areas first and the areas where ligaments insert into bone, using templates, Cold ultrasound and stretching techniques. Spine range of motion exercise routine taught to patients with an emphasis on getting to order intervals, focusing initially on the fight against gravity muscles such as thoracic and lumbar extensors. The rotation of the neck and chest retraction and rotations are also important to keep functional movements. Patients should rest in positions as good as prone or supine with one pillow to avoid accentuating the typical spinal deformities. Treatment of AS in a hydrotherapy pool is beneficial and soothing and patient education major, so keeping your program.
